Who owns my health information on the card?
You do! You have absolute control on the availability, completeness, accuracy, integrity, privacy, confidentiality, security, backups and access to your health data that is stored on the card. POLLYE Health does NOT store, sell or distribute any of your personal health data.
Do I have to fill out all the forms in a doctor’s office?
No. The POLLYE Health Card provides your Doctors with a clear integrated view of your health history. Your Doctor can also instantly print a structured summary of your health history (“health view”). Of course there may be other forms that your Doctor wants you to sign such as insurance etc, but when it comes to your health history, it is available and printable instantly for his/her viewing.
How much is the card?
Please refer to our pricing page for details. The POLLYE Health Card is 100% FSA and HSA eligible.
Are there any additional fees?
There are no additional fees for the POLLYE Health Card.
Can my card’s information be stored on another computer?
No. Your POLLYE Health Card will never leave your personal health information on any computer. We do however encourage you to backup your card regularly for safekeeping.
How long is my POLLYE Health Card good for?
The POLLYE Health Card is yours for life! If at any time your POLLYE Health Card is damaged, we will replace it for you at no extra cost.
How does my POLLYE Health Card get updated with new medical information?
The POLLYE Health Card gets updated after each Doctor visit. If your PCP is not a participating POLLYE Health Physician, our Board Certified Physicians will update it for you.
I already have Apple Health. Why do I need the POLLYE Health Card?
Medical errors happen due to missing, incomplete, or wrong information in your medical records. The POLLYE Health Card allows you to collect your medical history from ALL you treating physicians, regardless of the hospital, outpatient clinic or urgent care settings you visit. Unlike Apple Health, your health history is not dependent on one particular hospital or electronic health record company but collects info from ALL medical facilities and Physicians you visit in your life.
Does software need to be installed in order for me to view my medical information?
No. Your POLLYE Health Card comes complete with all the software so no additional software is needed. Just plug and view. In addition, no internet is needed. All the software is self contained in the card itself
Will my doctors need additional hardware or software to view my information?
No. Just give your POLLYE Health Card to the Doctor or Health Provider at your visit and S/he can view your medical history instantly from their computer
Does it take long for my doctor to view my health information?
Your information is available to your doctors in seconds and does not require an internet connection
What information is included in the initial Physician Activation?
The initial Physician Activation includes medical history that your PCP or POLLYE Health Physician deems relevant. It includes, but not limited to, medical diagnoses, list of medications, allergies, immunizations, and surgical history.
What happens if my POLLYE Health Card is lost?
Your POLLYE Health Card enables copies and backups of all the components of the package and thus prevents the serious problems that are created when the original copy is lost, damaged, or stolen.
Does my Pollye Health Card have all the info in the Electronic Health Record?
Your POLLYE Health Card contains medical information your PCP, Specialists and treating Physicians deem relevant. There may be times that additional information is needed and your Physician may request records stored in the EHR (Electronic Health Record).
What if my PCP is not a POLLYE Health Provider?
No problem, we’re here to help! We will have one of our POLLYE Health Physicians on staff activate and update your POLLYE Health Card for you.
What if my new doctor didn’t receive my medical records prior to my visit?
You just hand your POLLYE Health Card to your new doctor. All relevant data will be available instantly and in computer readable view for your new doctor to see.
What if I don’t have a PCP (Primary Care Provider)?
No problem! Contact us and we will have our Board Certified POLLYE Health Physicians activate the POLLYE Health Card for you and update it when needed.
Is the membership covered by my insurance plan?
The POLLYE Health Card is 100% FSA (Flexible Spending Account), HSA (Health Saving Account), and HRA (Health Reimbursement Account) eligible. Currently, insurance plans are not covering the POLLYE Health Card but we are working hard on your behalf for them to do so. Stay tuned!
What if I cancel my subscription after a year. Do I have to return my POLLYE Health Card?
No, your POLLYE Health Card is yours to keep forever. However, up to date medical information is very important at the point of care and we encourage you to maintain your POLLYE Health Card up to date at all times.
