SIGN UP FOR YOUR POLLYE HEALTH CARD

Purchase your POLLYE Health Card and complete the registration form online.

CARD ACTIVATED by a POLLYE HEALTH PHYSICIAN

Once you are signed up, a POLLYE Health Physician will contact you to activate your POLLYE Health Card. Initial activation includes populating your POLLYE Health Card with relevant medical information that includes, but is not limited to, your medical diagnoses, list of medications, allergies, immunizations, and pertinent test results.

YOUR POLLYE HEALTH CARD WILL BE SHIPPED DIRECTLY TO YOU

You will be notified when your POLLYE Health Card ships. Once you receive your POLLYE Health Card, it is ready for you to view and use. CONGRATS!
